Working Through Your Current Laboratory

(Local hospital, regional or national reference lab, etc)

  • Please contact the customer service department at your current laboratory to discuss the testing you would like IBT® to perform.
  • Please complete their test request form. Ask the lab (facility) for their test code that corresponds with the appropriate IBT test code.
  • To ensure the sample is sent to IBT, please write “Send to IBT Labs” in the comments section of the test request form.
  • If your laboratory is not a current client of IBT, please have them call our Client Services group at 1-800-637-0370 to set up an account.
  • IBT will bill the sending laboratory directly.

Working directly with IBT Laboratories

  • Please call IBT Client Services at 1-800-637-0370 to set up a new account.
  • We will send you the necessary paperwork enabling you to send your sample to us that day.
  • Once your first sample is received, you will receive a welcome packet with information about IBT, test request forms and a service directory CD.
  • Two Billing Options are available:

  • Monthly Invoicing: We will send a billing authorization form to you to set up monthly invoicing. Our billing terms are net 30 days.
  • Payment in Advance: Arrangements can be made for you to pay in advance for your testing services. IBT accepts check, Master Card, VISA or American Express.
